Initial Visit

Welcome!  Here is what to expect at your initial visit:

We ask for your assistance by providing the following:

• A picture ID
• Your insurance card
• A list of medications you are presently taking

Any patients under the age of 18 must be accompanied by a parent or legal guardian.

In the case of guardianship or a custody decree, you must bring a notarized proof of guardianship to the initial visit.

Location / Parking: We are located on Waianuenue Avenue, right above Hilo Union School.  Waianuenue Avenue is the street the Hawaii Public Library, Kozmic Cones and Hilo High School are on.  If you are coming up the street with the ocean at your back, we are on the left side of the street in a light grey building.  Our parking lot is behind the building and you may park in any of the stalls facing the beige apartments or behind any of the cars parked in the grass, those are employee vehicles.

Patient Forms: You may do your forms online by selecting the Patient Forms tab and selecting the New Patient Forms link.  There are 3 forms that need to be done and when you are finished they will automatically generate to our system, so there is no need to print them.

Insurance​: We accept most PPO insurances, unfortunately we do not accept HMO plans.  If you have an HMO plan, you need to contact your insurance company for a list of dentists that participate in the program.

We are happy to submit to your insurance on your behalf and estimate co-payments, but please keep in mind that it is your insurance and being so, the insurance company will not inform us of any changes to your plan or eligibility.
